Job Description

Bluebird Care are now partnering with Somerset NHS trust to support patients back into their own home and to be able to live independently.We are looking for Rehabilitation Care Workers to join and help grow our brand-new team in Bridgwater on a full-time basis. You will also be required to travel around the Sedgemoor area, so you must have a Full Driving Licence in order to be considered for this role.Within this role you would be working with customers and the hospital OT teams to enable customers to be back in their own home and living as independently as possible. Visiting the customers in their homes you will be working to support them in their day to day life and achieve the goals set for them.Requirements to be a Rehabilitation Care Worker:

Someone who is able to assess the customer needs & support with care whilst promoting independence and rehabilitation

Decision making skills are key for this role

Attention to detail is necessary with the writing of care plans and reports

This role has a 2 week shift pattern of 1 week 7am to 3pm and the 2nd week 2pm to 10pm with alternate weekends. Flexibility will be needed from candidates for the first couple of months for the induction and training period. After this time, the working hours will be based on a 7am- 3pm or 2pm-10pm shift.This role is paying £11.50 per hour which equals up to £23,920 per year. Required skills
