Band 6 Occupational TherapistBased at Perry Tress Care Centre (north Birmingham)Role:Assess new patient admissions, where possible jointly with physioIdentify short term goals, achievable within 2 weeks.Devise a treatment and discharge plan with physio in accordance with Discharge 2 Assess model.Liaise with patients and relatives about discharge plans.Attend daily MDT tracker meetingsComplete necessary access visit assessmentsPrescribe and order equipmentRefer to community therapy- Early intervention community teamDevise plan care plan for requesting package of careRefer to social worker if requiredMental capacity assessmentLiaising with out of area local authority and ordering equipment and community therapy.Support Band 5 with caseload, report to Band 7 with any complex casesAbility to work cross sites - car driver essentialPlease contact Alex for further details
