Job Description

Complex Adults Team LeaderLOCATION: BathHOURS: (37.5 per week)SALARY: £26,520DURATION: PermanentSTART DATE: ASAPAdditional information:The successful Team Leader will have day-to-day responsibility for service delivery. You will lead and inspire staff and ensure that they are supported to deliver the highest standards of service delivery to clients.Main Activities and responsibilities for the Role:

  • Provide strategic leadership for a new service that is designed to provide intensive, person-centred support and reduce rough sleeping in Bath and North East Somerset.
  • Co-ordinate services around client need and ensure full communication between services; providing a leadership role and ensuring maximum take up of support including single service plans
  • Lead the multi-agency working with external agencies required to reduce rough sleeping, including cross authority.
  • Complete new and review, needs assessment/risk assessment for clients leading to the delivery of full support plans which meet individual client need
  • Track and monitor support delivered and its impact through review and use of outcome tools, with a particular focus on trauma informed care and gender informed interventions.
