Part Time 22.5 hours per weekWe are looking for a Care Transfer Co-ordinator who is motivated, enthusiastic, individual to work in collaboration with the multidisciplinary teams in facilitating the seamless transfer of patients from nominated District General Hospitals to the patient’s own home, residential home, nursing home or a community hospital.The base for the job is Ystradgynlais Community Hospital, and this will be the place of work most days, where you will have the opportunity to work alongside colleagues from Powys and Swansea Bay University Health Board to ensure our patients receive the most appropriate care in the most suitable location. The working hours are core hours, Monday-Friday, 22.5 hours over five days.The post holder must be a Registered Nurse and hold relevant qualifications with their professional body.We have a clear "Home First" ethos and we are looking for someone who is keen to develop and learn new skills. We want someone who is caring and sympathetic, an excellent communicator and has an attitude that ensures the highest standard of care for our patients . The work is fast paced, interesting and extremely fulfilling, you will be making a difference every day to patients and their families - no two days are the same. You will receive support from other Care Transfer Co-ordinators and the Senior Nurse, Unscheduled Care.
