Job Description

This role has a starting salary of £29,291 per annum based on a 36 hour working week.Do you excel in your profession and are you interested in the challenges of working in a busy, professional environmentA Mental Health Hospital Discharge team has been in operation on a temporary basis since April 2020. Due to its pivotal role it has been agreed that the service becomes a permanent team within the ASC Mental Health structure.The Mental Health Hospital Discharge team supports all discharges for Surrey patients in SABP acute mental health hospital bases who have care and support needs. It operates across working aged adults, older people (functional and organic needs) and adultswith young-onset dementia (YOD).The team works with the person to facilitate safe and timely discharge and in the persons best interests; supports them to return home or to a community placement.The team works on a principle of home first so that support is provided to the person athome and in their community using residential and nursing care as a last resort.The service works using strength-based practice, listening to and working with MH service users as to how they would like their assessed needs met in the community with support and access to family, friends and inclusive community support.This dedicated team is working closely with SurreyandBordersPartnershipNHS (SABP) to ensure there are coordinated, robust processes across the mental health system to enable the achievement of enhanced health and social care outcomes in a flexible wayand which minimise delays in the system.This team operates in the usual locality team hours between 9am-5pm with EDT taking work at 5pm as now.The post holder will be expected to travel across Surrey to the various hospital sites. The base for the Mental Health Hospital Discharge team is Quadrant Court, 35 Guildford Road, Woking, GU22 7QQ.The role involves a mixture of office, hospital and community based work. There will be excellent training opportunities on strength-based approaches and the role will be a real opportunity to shape the working of the mental health teams and really makea difference in ensuring safe and timely hospital discharges.In these roles:

  • You will need to be motivated and flexible with high professional standards and sound knowledge and experience of current legislation, including the Care Act and the Mental Capacity Act and its application to practice in a social care setting
  • You will need to be confident in undertaking safeguarding enquiries, carry out assessments within hospital settings and the community and working with a diverse group of individuals and partner agencies
  • You will be able to communicate in a clear, fluent, concise and jargon free way and in a courteous, calm and professional manner.
