The starting salary for this role is £41,127 per annum,based on a 36working hour week.The Transition Team, based in Woking or Redhill, is a dynamic and vibrant countywide service for young people aged between 14 and 25. Although we sit in Adult Social Care, our journey with the people we support starts much earlier at academic year 9. WithinSCC, the Transition Team sits in a unique and favourable position of being able to undertake frontline work across both Children and Adult Services.We work with young people who have a wide range of additional needs and complex conditions. We challenge inequality and traditional responses to service delivery and ensure that we employ a strength-based approach to our work. This facilitates a more innovativeapproach to our support planning processes thereby realising more outcome focused goals for the young people we support and their families.We believe in coproduction and feel passionately that the young person should be firmly in control of their life outcomes. Supporting them through the Preparation for Adulthood agenda, we also ensure that our young people have aspirational goals and targetsthat will support them into employment pathways, independent living and all other meaningful occupation.How is working within the Transition Team a rewarding experienceWorking in the Transition Team, you will benefit from a rich tapestry of experience. We work with a wide range of professionals and providers who enable us to look at an individuals needs holistically and consider our service response from a whole family perspective.We are looking for enthusiastic and dynamic qualified and registered Social Workers. For this role you will need to have completed your Assessed and Supported Year in Employment (ASYE). We recognise there will be exceptions to this so please speak to usif you are in any doubt.We are looking for social workers who believe it is important to ensure people have choice and control over their own support needs. We want people who are committed, like us, to focus on the person, their strengths and networks rather than routinely assessingpeople for services and eligibility. We are currently rolling out hybrid devices whichhave a touch screen, detachable keyboard and will enable staff to access systems such as LAS when working with clients. The new devices will allow colleagues to change the way they work and to help avoid unnecessary travel.
