  • To provide specialist physiotherapy assessment, exercise prescription and management for those patients referred to the Community Therapy and Falls Team.
  • To provide an evidenced based assessment and treatment of patients who may have complex and/or chronic presentation, provide physiotherapy treatment and maintain records as an autonomous practitioner.
  • To assist the Team Leader in planning, co-ordinating, delivering and evaluating the physiotherapy service provided to on a day-to-day basis.
  • Function as an effective member of the multi-disciplinary team and the integrated team within Health and Social Care.
  • Assist in the identification of health needs
  • Participate in the delivery and evaluation of care interventions

  • Diploma/Degree in Physiotherapy
  • HCPC Registered Physiotherapist
  • Degree in Physiotherapy
  • Evidence of relevant post graduate Courses/CPD
  • Varied experience in working with the elderly and Falls
  • Able to communicate with individuals and groups at different levels and variety of settings
  • Familiar with a variety of outcome measures and interventions used within the Falls and community setting
  • Ability to work independently
  • Able to work collaboratively within the multidisciplinary team

  • Knowledge of a wide range of conditions and their physiotherapy management
  • Advanced clinical knowledge in Falls

What will be your main responsibilitiesKey results from the job holder
  • To be professionally and legally responsible and accountable for all aspects of your own work including the management of patients in your care. To ensure high standards of clinical care for the patients under your management, and support more junior staffto do likewise.
  • Post holders are required to work independently, guided by defined clinical & professional guidelines. Access to a more senior member of physiotherapy staff is available when necessary. Clinical work is evaluated on a regular basis.
  • To interpret and analyse clinical and non-clinical facts to form accurate assessment and risk stratification in a wide range of conditions, to recommend the best course of intervention, and to develop comprehensive discharge plans in regard to the patientsself-management.
Planning and Organisational Duties
  • To accept clinical responsibility for a designated caseload of patients, and to organise this effectively and efficiently with regard to clinical priorities and use of time.
  • To accept partial responsibility for any tasks delegated to less senior qualified or unqualified staff.
  • To assist the team leader in developing policy changes within the speciality that will impact on service users. To assist the team leader in ensuring that designated staff implement policy and service development changes.
  • To work with the team leader in developing the operational management of the Community Therapy and Falls Service.
  • To provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals, to promote understanding of the aims of physiotherapy, and to ensure a consistent approach to patient care.
  • Supervise, educate and assess the performance of physiotherapy students; this would be to a graduate standard and involve working with Universities to ensure the standard of practice and teaching meets the standards set by the degree level qualification.
Communications and Key Working Relationships
  • To communicate effectively with patients and carers to maximise rehabilitation potential and to ensure understanding of condition.
  • To use communication tools (verbal and non-verbal) to gain informed consent with patients where there will often be barriers to effective communication e.g. loss of hearing, anxiety.
  • To assess capacity, gain valid informed consent and have the ability to work within a legal framework.
