Role: To support individuals in the local community to live independent, fulfilling and enriched lives. As a support worker, you will manage a case load and be responsible for supporting people with Mental ill health, Learning disabilities and Autism who live in the local community. Although you will be responsible for supporting people with their everyday activities this excludes personal care. You will build trusting relationships and have good rapport to be able to support individuals to reach their full potential and reach outcomes. you will be responsible for paperwork including, daily reports after each contact, Support plans compiled with service user involvement and Risk Assessments.Activities of support vary from daily living skills, support to appointments, travel, socialising completing forms and accessing community-based activities such as going to gym, walking, community groups, work placements and other social and leisure activities and supporting them through difficult periods in their life. No day will be the same. Some people may need emotional support to get them through a difficult time, while others will need a more hands-on approach.
