Job Description

Qualified Social Worker - Learning DisabilityLocation: HammersmithSalary: £38,037 - £45,711Contract: Permanent, Full-TimeAre you a passionate Social Worker ready to make a difference Join our vibrant team in Hammersmiths Adults Learning Disability Team. Were seeking a qualified and dedicated professional to support adults with learning disability, empowering them to liveindependent and fulfilling lives.Responsibilities:To engage with individuals, carers, and families to assess and intervene, and using professional judgement, employ a range of interventions promoting choice, control and independence.They will ensure that as part of the assessment in accordance with s.9 and s13 or the Care Act 2014, NHS Continuing Healthcare is considered for all individuals, and where it is considered, an individual has health care needs, a referral to the relevanthealth authority is made in a timely way.They will also assist with the duty function of the team as required, providing immediate professional guidance on urgent issues, arranging immediate support to manage risk in line with a personalised approach and escalating issues of concern to more seniorstaff.

  • Learning Disabilities/ Mental Health social work staff working in Adult Social Care will be based within the specialist services function.
  • They will be required to work flexibly and will be supported to do this with mobile IT equipment and hot desk facilities in all offices across the borough.
  • Reasonable adjustments will be made for social workers who have a disability/mental health requirement to enable them to fulfil the requirements of the job.
  • Professional, practice-based supervision will be provided.
  • May make recommendations for the provision of services in line with the budget determined according to the assessment of needs and advise less experienced or unqualified colleagues on budget and the cost of services
Requirements:
  • Qualified Social Worker with SWE registration.
  • Experience working with adults in a Social Work environment.
  • Sound knowledge of adult social care legislation.
  • Strong assessment and communication skills.
  • Ability to work in a multidisciplinary team.
