Job Description

Community Support Worker/Social NavigatorSalary: £12,012 per annum for 21 hours per week over 3 days per weekAre you looking for a part time opportunity 3 days per week .. if Yes! read on!Do you DRIVE and have good listening and empathy skills to deal with bereaved carers... If this is yes, then ProSkillz LTD may have the perfect opportunity for you!ProSkillz Ltd are working in partnership with a West Midlands based Charity to recruit a Community Social Navigator to provide one to one wellbeing support tailored to the needs of bereaved carers in their own home or in the community.The Social Navigator will be required to work with clients to complete their initial needs assessments, support plans and to work with people on an individual basis in a person-centred manner. You will be responsible for maintaining your client files, collectingmonitoring data including evidenced based well-being data. You will be required to attend Caring for Carers team meetings and training as required.The main duties include:

  1. You will be required to work for Caring for Carers supporting Carers through bereavement and loss.
  2. You will deliver weekly support interventions to all clients where appropriate.
  3. You will carry out weekly telephone wellbeing checks on all clients.
  4. You will undertake one to one outreach visits with all clients where needed.
  5. Encourage Signposting within Caring for Carers services such as Healing Gardens, Living Memory connecting carers to other bereaved carers.
  6. Undertake casework for clients, family members and carers as relevant. Case Manager Software will be used to record client data.
  7. To maintain computerised and written records to maintain and collect monitoring data for clients including attendance, signposting, and a range of individual outcomes.
  8. To be aware of and respond to the needs of clients reporting concerns about ill health and any safeguarding concerns to team members and/or senior staff and treat these concerns with urgency.
  9. Provision of information, advice, and guidance sessions for users of the service.
  10. Publicising activities and events within the centre via social media and other means, with guidance.
  11. To participate in fund-raising events, awareness raising of mental health issues e.g. World Mental Health Day, Black History, Heritage and Carers week, Mental Health Awareness week etc, and other related initiatives.
  12. Any other duties that fall within the parameters of the post
