Job Description

Dual Needs Homelessness NavigatorFull time, Fixed term contract (12 months)£22183 per annumBased in Wellingborough, covering the North Northamptonshire areaJOB PURPOSEThe purpose of the Dual Needs Homelessness Navigator is to proactively engage with clients who are currently or formerly rough sleeping across the North Northamptonshire area. Adopting a person-centred, outcomes focussed approach, you will work collaborativelywith key strategic partners to deliver high quality, integrated services that facilitates clients access to drug and alcohol treatment, mental health services, and housing support. You will build trust, provide personalised packages of flexible, responsivesupport whilst enabling clients to navigate treatment pathways and progress towards a life away from the streets and sustainable living.KEY RESPONSIBILITIESSupporting Clients

  • Undertaking client risk and needs assessments and developing client led, outcomes focussed support plans to assist clients to achieve their outlined goals.
  • Building professional relationships with clients to understand and to support with any barriers to accessing support or appointments.
  • Working closely with Drug and Alcohol teams to ensure that clients have timely access to treatment and to support their continued engagement with services.
  • Working closely with Mental Health services to ensure that clients have access to address their mental health needs for both diagnosed and undiagnosed conditions.
  • Working closely with the Rough Sleeping Team to identify and prioritise where support is required and to positively contribute to tenancy sustainment of clients
  • Proactively engaging with clients to offer a range of flexible and effective interventions to provide holistic wrap around support.
  • Encouraging and assisting clients to establish positive social networks and undertake meaningful activities to assist and promote social inclusion.
  • Working with individuals to build community ties through the engagement opportunities of volunteering, training, or employment with a view to moving them towards becoming economically active and empowered.
  • Completing referrals to other specialist organisations or services where appropriate.
  • Securing qualitative or quantitative client feedback, ensuring our services are relevant and responsive to changing client needs.
Partnership Working
  • Identifying and understanding the full range of support services available in each locality to ensure that rough sleepers can be signposted to services that enhance their support.
  • Working closely with all key partners, sharing appropriate information, and ensuring action plans are carried through.
  • Working collaboratively and effectively as part of a multi-agency response to reduce street homelessness and increase sustainment of accommodation.
  • Assisting and contributing to best practice developments in providing rough sleeping initiatives and solutions to homelessness.
  • Supporting and promoting the provision of programmes aimed at reducing rough sleeping and improving client access to treatment services.
  • Supporting the delivery of the Severe Weather Emergency Protocol in accordance with local policy and national guidance.
  • Periodically supporting the delivery of local Rough Sleeper Counts including at night.
  • Attending and contributing to multi-agency meetings and/or multi-disciplinary case conferences.
Managing Data, Monitoring, Reporting and Impact
  • Meeting performance targets, including maintaining accurate and comprehensive client records both electronic and paper based as appropriate.
  • Maintaining accurate up to date client records, ensuring that all interventions and outcomes are recorded on the agreed management systems.
  • Monitoring performance information and providing regular updates including weekly, monthly, or quarterly reports as required.
  • Conducting ongoing evaluations of support programmes and services, identifying changing trends or gaps in service provision, and making service improvement recommendations.
  • Reporting all incidents or highlighting areas of concern to the Chief Executive.
General Tasks
  • Creating positive relationships and treating all clients, staff, volunteers, and key stakeholders with dignity and respect, demonstrating an understanding of the Daylight Centres core aims and values.
  • Adhering to all DCFs policy and procedures
  • Complying with all health and safety requirements, taking reasonable care for the health and safety of yourself and others.
  • Adhering to and recognising personal and professional boundaries at all times.
  • Responding appropriately to any safeguarding concerns, demonstrating an understanding of and compliance with DCF safeguarding procedures.
  • Managing complaints processes, resolving issues in line with procedures.
  • Engaging in bi-monthly supervision and performance meetings with the Chief Executive.
  • Fully preparing for and engaging in annual performance review processes, agreeing future targets and objectives.
  • Undertaking training identified as necessary for the role and taking ownership of continued professional development.
  • Keeping abreast of developments in services, legislation, and relevant practice in relation to rough sleepers, housing, substance misuse and mental health.
