Join us now and you can make a real difference to people’s lives.As a Care Coordinator with Hertfordshire Community NHS Trust (HCT), you will be supporting parents/carers of pre-school children with complex needs, empowering them to understand and navigate the different systems their children require access to.With some previous experience of working with health professionals, parents and/or children, you will be involved in home visits, multi-professional meetings, phone and video liaison as well as providing emotional support to parents/carers which is a key part of the role.As you will provide some admin help to families such as support with applications and managing appointments, you will need a good working knowledge of a variety of computer systems and programmes.With the choice of being based at Watford or Stevenage as part of this pilot project, as well as being part of this friendly but professional team, your continuing professional development will also be actively supported through access to training, regular supervision, peer support meetings and staff appraisal scheme.Although currently fixed term for the duration of the project, it is anticipated that this role will be made permanent.Please make your application now as we are expecting a lot of interest in this new role.
