Job Description

Tripod Partners are seeking a skilled and compassionate Social Worker to join our Adults Mental Health Team in Walsall. As a valued member of our mental health services, you will play a pivotal role in supporting and advocating for individuals experiencingmental health challenges. This position offers an exciting opportunity to make a positive impact on the well-being of adults within our community.Key Responsibilities:

  1. Assessment and Intervention:
    • Conduct comprehensive assessments of adults with mental health concerns, evaluating their needs and strengths.
    • Develop and implement person-centered care plans, collaborating with service users and their support networks.
  2. Case Management:
    • Provide ongoing case management, ensuring effective coordination of mental health services and interventions.
    • Monitor and review care plans, making adjustments as necessary to meet the evolving needs of service users.
  3. Crisis Intervention:
    • Respond promptly to crisis situations, providing immediate support and coordinating appropriate interventions.
    • Collaborate with crisis intervention teams, healthcare professionals, and emergency services as needed.
  4. Collaboration and Networking:
    • Work collaboratively with multidisciplinary teams, including psychiatrists, psychologists, and community resources.
    • Establish and maintain partnerships with external agencies to enhance support options for individuals with mental health needs.
  5. Advocacy and Empowerment:
    • Advocate for the rights and choices of individuals with mental health challenges.
    • Empower service users to actively participate in decision-making processes regarding their care and treatment.
  6. Documentation and Reporting:
    • Maintain accurate and up-to-date case records in accordance with organizational standards.
    • Prepare detailed reports for internal reviews, external stakeholders, and legal proceedings.
Qualifications and Requirements:
  • Bachelors or Masters degree in Social Work or a related field.
