We have an exciting opportunity for a Care Coordinator based with our Care Connection Team (CCT) in Hillingdon.The position is for a fixed term 12 months contract to cover Maternity Leave .Hillingdon Health and Care Partners (HHCP) is focused on delivering integrated care across its providers with a wide range of partner organisations and stakeholders, including acute trusts, mental health trusts, local authorities, community services and primary care, as well as voluntary sector organisations.The Care Connection Teams (CCTs) will provide integrated care planning (ICP) and active case management to our over 18 patient population in a “whole systems” approach with the aim of reducing hospital admissions and caring for patients in their own homes where possible. There are 8 CCTs based within various GP practices across the borough supporting a group of allocated practices and their patients.We are looking to recruit a Care Coordinator to work as an integral part of the Care Connection Teams working closely with and supporting the Guided Care Matrons to actively case manage patients whose condition is at risk of becoming unstable, whose needs have escalated or who would benefit from a proactive care planning approach to reduce the risk of deterioration of health.The successful candidate will be working with colleagues in general practice to identify appropriate patients and be the main point of contact for patients and their families/carer in relation to the care plan and signpost and liaise with multi provider agencies including the voluntary sector .Ideally, you will have experience in working in a health or social care environment and have coordinated a range of services using different organisations in a multi-professional environment. Your empathic approach will enable you to communicate effectively and talk to people about sensitive issues in a supportive manner. The ability to understand various databases is vital to this role as you will need to navigate within different platforms (such as EMIS and System One) in order to risk ratify and identify the right patients to be discussed at huddles and eventually if appropriate , will be case managed. Therefore, candidates should be proficient in IT skills.The contracted hours for this position will be Mon - Friday , 9am-5pm. No weekends or Bank Holidays.Although this position will be based in general practice, Central and North West London NHS Foundation trust will be the employer and staff member will be on NHS terms and conditions.
