Continuing Healthcare Specialist Nurse Assessor 30 hours per weekThe opportunity has arisen for the permanent post of a Continuing Healthcare (CHC) Specialist Nurse Assessor Band 6 to work within the CHC Hub. The post holder will be responsible for the specialist Continuing Healthcare assessment of the health needs of individuals either in their own homes, residential or nursing accommodation. They will also manage the Fast Track process and complex care management for clients eligible for Continuing Healthcare ensuring that all health needs are fully met.The post holder will be expected to work with the wider multi-disciplinary team and they will require analytical assessment skills and have excellent inter personal skills in order to undertake a high standard of assessment. This is an excellent opportunity for nurses to gain knowledge and understanding of CHC and enhance their assessment skills whilst being part of an experienced and supportive team.
