Job Description

Do you want to make a difference to the people of Somerset at a time of their lives when they need it mostAre you someone with a background of care and education We have an exciting group of posts to suit all geographies of Somerset to help an established and highly regarded county wide End of Life care service providing outstanding care to people in their last days of life. You will join an integrated and multidisciplinary team as an educator and facilitator of excellent care. The post holder will improve care in the last days of life through a coordinated education approach across Somerset care providers as part of the CCG funded integrated approach to End of Life care (EOLC). Improve our culture, communication and collaboration to enhance the care of those in their last days and those important to them. The role will have bases in both acute trusts and also in the community. It would suit anyone with an interest and skill in the subject area who lives in or near Somerset. Core Team Purpose: We promote attention to the achievement the person is still able to make in the face of physical deterioration. We recognise the dying person’s need for comfort, support, dignity and encourage them to move toward whatever degree of completion and acceptance they are able to attain in their life regarding themselves, their family, their relationship with society, and their individual conception of life’s spiritual basis.
