Service Support WorkerThis role is working for The British Red Cross, who help people in crisis, whoever and wherever they are. Within the UK, the British Red Cross operates through over 3000 staff and over 20,000 volunteers.The Independent Living & Crisis Response service, supports people in the space between hospital and home and responds to the needs of people in crisis following an emergency; maintaining an extensive network of external relationships across health, social care, and emergency services.This is a temporary contract to the end of the year with the chance of extension.You must be over 21 with a full Manual Driving License, and a full, clean Enhanced DBS.Overall purpose of the roleThe Independent Living Service Support Worker post will provide reactive, focused and flexible support to adults either following a hospital stay to enable early discharge when clinical needs have been met or to those living in the community to prevent hospital admission. The post will assist the Independent Living Service Coordinator by undertaking duties required to meet the service users’ needs in order to re-enable service users to remain in their own home and prevent hospital re-admission. Referrals may require the support worker to take the patient home from hospital to enable their discharge. The post-holder will respond to referrals from Clinicians and Community Health Professionals and will be required to have the ability to assess service users’ needs, complete and follow a support plan for the individual. The post-holder will be providing practical and emotional support in the service user’s own home and signposting to other services where appropriate to enable the individual to live independently in their own home.Principal responsibilities
