Job Description

An exciting opportunity has arisen through the Primary Care Networks to deliver an adult community nutritional support programme within the Sunderland locality. This is a new and innovative Post, with a real opportunity to make a difference and develop a genuinely primary care-led dietetic service in Sunderland.We are therefore looking for a Dietitian who has a keen interest in community dietetics to provide specialist nutrition and diet advice to patients, their carers and healthcare professionals to assess and treat conditions, manage long-term conditions, as well as promote self-care.As part of the role you would be responsible for the set-up, management and audit of the Programme. You would be part of the delivery process engaging GPs and practice nurses to refer into the Programme and providing essential advice and ongoing support to both patients and colleagues. You will be required to work with clinicians, MDT colleagues and external agencies to ensure the smooth transition of patients discharged from hospital back to primary care.This role will be working autonomously alongside a Band 7 colleague and will be a patient-facing, involving both community-based clinics and domiciliary visits. You will be expected to work autonomously, assessing, planning, implementing and evaluating dietetic interventions in order to prevent unnecessary admissions, ensure safe discharge and provide rehabilitation in the community. This includes providing specialist nutritional assessment advice and appropriate prescribing of oral nutritional supplements.You will also be responsible for feeding back the project results to the Primary Care Network regarding outcome measures both quantitative and qualitative.You will be employed as part of South Tyneside and Sunderland NHS Foundation Trust and have access to support from Dietetic colleagues and Clinical Supervision. Our Dietetic Department of more than 50 staff is managed as one team, but is structured into smaller teams led by Senior Specialist Dietitians.
