Lead Support Worker Hospital Discharge Sunderland & South TynesidePermanent Full Time & Part time hours available£21,000 (FTE) plus, brilliant Benefits!Empowering customers to live empowered and happy lives. You will be uniquely placed within the hospital multi-disciplinary teams; tasked with helping patients from the point of hospital admission to discharge overcome housing related barriers. We will be able to continue that support when customers move on intotheir own homes, and will be integrated into the crisis team, providing housing related support to help reduce readmissions.
