Lead Support Worker Hospital Discharge Sunderland & South TynesidePermanent Full Time & Part time hours available£21,000 (FTE) plus, brilliant Benefits!Empowering customers to live empowered and happy lives. Thats when it hits home.We have fabulous opportunity for you to be part of the brand-new Hospital Discharge and Crisis Team! You will be uniquely placed within the hospital multi-disciplinary teams; tasked with helping patients from the point of hospital admission to discharge overcome housing related barriers. We will be able to continue that support when customers move on intotheir own homes, and will be integrated into the crisis team, providing housing related support to help reduce readmissions. You could do this using public transport, or if you drive and have access to a vehicle that would be amazing (we pay your expenses to cover thisuse). And as you will be working within NHS wards all colleagues must be fully vaccinated against COVID 19....
