Heart Failure Specialist NursePart Time 18.75 hoursWe have an exciting opportunity for an experienced Cardiac Nurse to join our well established Heart Failure Team at South Tyneside and Sunderland Foundation NHS Trust working closely with the Consultant and Pharmacy lead. As part of this team you will provide specialist heart failure care to patients admitted with heart failure and provide ongoing care through a nurse-led clinic, or onward referral to the Community Heart Failure Nurses.Referrals are also received from Consultant clinics for up titration of heart failure medication, education and support for patients with heart failure and also for admissions avoidance. Case management of these patients is an integral part of the role. In addition education is provided to both secondary and primary care staff.You will need to be an experienced Cardiac Nurse with an interest and expertise in heart failure care, although full support and training will be provided. To this end you will need to be a degree level nurse who has had significant experience of caring for heart failure patients, with the ability to work both independently and as part of a team. You will need to have good time management and organisational skills, along with experience of clinical audit to demonstrate effectiveness of the role.Aims of the role are to:
