Job Description

Position: Support Worker (Twilight)Location: Stoke-on-TrentHours: Part time - 18.00 - 00.00 on a rolling rota to include weekendsSalary Details: £9.65 per hourWe are looking to appoint a TwilightSupport Worker to join our friendly, established care team to provide high quality care and support for our patients at Oakwood Supported living.Essentials:

  • Full UK Driver’s license with own car
  • NVQ/QCF Level 2 Health and Social care
  • Experience in a care setting preferred but not essential
  • Experience working with young people and/or adults with Learning disabilities, Autism and mental health problems
  • Roles and Responsibilities:
  • As a Support Worker you will provide direct patient care and support as part of a multi-disciplinary team to a group of patients within a hospital service.
  • You will take the lead role in task and resource allocation on a day to day basis under the professional guidance of the Senior team, supporting each patient in achievement of their planned outcomes.
  • You will assist the team with the assessment of individual patient / resident needs, planning, delivery and evaluation of care for a number of individuals or groups of patients & following appropriate training
  • It is important for our tenants to have every opportunity to enjoy the benefits of community living whilst providing a high quality safe and secure environment that is positive and supportive and patients are encouraged to get involved with social activities within the service to help them on their recovery journey.
  • The staff at Oakwood provide support that is individually tailored to the tenants needs which is underpinned by holistic support plans which is essential for the tenant’s progress and greater independence.
  • For those living at Oakwood it is important for the staff to build trusting and therapeutic relationships with tenants in order to allow them to safely reach their optimum level of independence.
  • This is not an exhaustive list of duties and responsibilities, the post holder may be required to undertake other duties following appropriate discussion with their line manager.Benefits:
