The Falkirk Partnership is seeking to introduce a Housing Services CPN post. The post holder will be based with the Housing Needs Team in Falkirk Council. The post holder will provide support to individuals with mental health by providing an outreach service to the housing/homeless establishments within the Falkirk Council Area. Within Falkirk Council and NHS Forth Valley area there is a commitment to tackling inequalities and a strong focus inclusion.We are seeking a dynamic and forward thinking Registered Mental Health Nurse. Experience working in a community mental health setting and substance misuse would be advantageous. The post holder will have a focus on sustainable and high quality service delivery and will be critical to the embedding, development and evaluation of this exciting new role.The post holder would work across the Falkirk Partnership and will interface across NHS Forth Valley, Housing Services, Substance Misuse Services, SPS and other agencies to provide an accessible and responsive service to meet the mental health needs of individuals, thus helping to support the individual’s engagement and ultimately improve outcomes.Ability and willingness to travel throughout the area of service delivery is essential for this post.The duties of this post require the successful candidate to be a member of the Protecting Vulnerable Groups (PVG) Scheme.
