  • Advanced Apprenticeship in (Mental) Health and Social Care or NVQ Level 3 in (Mental) Health and Social Care with demonstrable evidence of numeracy and literacy skills (equivalent GCSEs or functional skills)
  • Experience of working with people with Mental Health Needs non-Dementia related

What you will be doing:
  • Working in an identified adult community mental health pathway covering 24 hours per day, 365 days per year, with the flexibility of working across pathways, dependent on skill and service needs.
  • Delivering a high-quality clinical service to patients and their families comprising:
  • Accompanying B6 staff in the assessment of crisis situations
  • Assisting with the provision of effective home treatment to prevent hospital admission including effective physical health assessment and care
  • Working collaboratively with other Trust services to provide seamless care for service users on defined care-pathways
  • Responsible for working in partnership with agencies external to the Trust supporting service-users access to services in the independent and non-statutory sector.
Under the direction of the senior health and social care staff in the team and/or the shift coordinator; deliver interventions with an allocated group of service users with mental health needs. The interventions will be focused on assisting the person toidentify and achieve their agreed goals of Recovery. These will include interventions related to; meaningful occupation, accessing employment, assessing and promoting physical health, social inclusion, activities of daily living, and mental health promotion.The post holder will also be involved in supporting the service user to complete appropriate outcome measure tools and participate in reviews of the service users care and the effectiveness of the interventions provided.IMPORTANT INFORMATION about your application.
