Job Description

Service Support Worker - Assisted Discharge Location: Bournemouth and surrounding area (Royal Bournemouth and Poole Hospitals) Contract type: Fixed term contract to the end of March 2022 Hours per week: 22 hrs part-time working x3 days flexibly between9am and 5pm as part of a 7 day service (Includes some weekends as per rota) Salary: £18,618 per annum pro rata (£11,702 per annum at 22hrs per week) Weve got an opportunity... We are looking for a passionate Team Supporter Worker to join our Royal Bournemouthand Poole Hospital Assisted Discharge Service. The Independent Living Service Support Worker post will provide reactive, focused and flexible support to inpatients following a hospital stay to enable early discharge when clinical needs have been met. The postwill assist the Independent Living Service Coordinator by undertaking duties required to meet the service users needs in order to re-enable service users to remain in their discharge location and prevent hospital re-admission. Referrals may require the supportworker to take the patient home from hospital to enable their discharge. The post-holder will be required to have the ability to assess service users needs, complete and follow a support plan for the individual. The post-holder will be providing practicaland emotional support in the service users discharge location and signposting to other services where appropriate to enable the individual to live independently in their accommodation. The successful candidate will hold a driving licence and have access toa car. The Support Worker will act as contingency for the Service Coordinator when required and may need to provide cover for their colleagues at other hospitals served by the Red Cross in the area. Service confidentiality and safeguarding vulnerable peopleare key responsibilities of the role. The post requires weekend shifts. This post does not include personal care and management of medication. Say hello to the team... The Independent Living (IL) & Crisis Response (CR) service (IL/CR)supports people in thespace between hospital and home and responds to the needs of people in crisis following an emergency; maintaining an extensive network of external relationships across health, social care, and emergency services. This role could be for you if... In order tobe successful in your application you must be able to demonstrate experience of working with vulnerable adults. You will also demonstrate that you are a good communicator who can delegate and manage set tasks to agreed timescales. Furthermore, you will haveexperience preparing and keeping written records and reports. You have ability to work with NHS professionals, community organisations, care agencies and relatives in order to support people in the community to life safely and supported at home. The closingdate for application is 23.59hrs on Sunday the 17th of October 2021 with interviews to follow. This role includes working face to face with service users and social distancing will not be able to be maintained at all times therefore, it may not be suitablefor people that may be at higher risk of harm from Covid-19.
