Service Support Worker - Assisted Discharge Derriford Hospital (must be able to travel within the greater Plymouth area) 21 Hours per week (3 set working days) Fixed Term Contract until 31 March 2022 Salary: GBP 16,217 per annum (pro rata) Weve got an opportunity... To support the provision of a home from hospital Assisted Discharge Service for suitable patients who are being discharged from Derriford Hospital to homes across the Greater Plymouth area. At the British Red Cross, we put the people that need us at the heart of what we do. As a Support Worker, you will provide the support and care which can make a vital difference to the quality of peoples lives, transporting them home in Red Cross vehicles and allowing them to continue living safely at home. You will provide reactive, focused and flexible support to adults following a hospital stay to enable early discharge when clinical needs have been met. You will assist the Independent Living Service Coordinators by undertaking duties required to meet the service users needs in order to re-enable service users to remain in their own home and prevent hospital re-admission. You will be providing practical and emotional support in the service users own home and signposting to other services where appropriate to enable the individual to live independently in their own home. This service operates Monday - Friday, between 9am - 6pm. The working days for this role are expected to be: Monday, Wednesday, Friday. Say hello to the team...The Independent Living (IL) & Crisis Response (CR) service (IL/CR) supports people in the space between hospital and home and responds to the needs of people in crisis following an emergency, maintaining an extensive network of external relationships across health, social care and emergency services. You live in the Plymouth area and have a local knowledge of the geographical area. Also, you should be the following: IT literate Full driving licence holder (manual licence essential) Have a good knowledge of services provided by the NHS and Social CareClosing date for application is 23.59 on Wednesday 26th May 2021.
