Service Support WorkerLocation: Warmley and Bristol area Contract: Permanent Hours: 17hrs per week, flexibility available for your working hours, 10am to 6pm Monday to Friday (flexible) Salary: £16,217 per annum, pro rata (£5,301 for 17 hours per weekper annum) Weve got an opportunity... As a Service Support Worker, you will provide the support and care which can make a vital difference to the quality of peoples lives,allowing them to continue living at home in comfort. You will provide reactive, focused and flexible support to adults either following a hospital stay to enable early discharge when clinical needs have been met or to those living in the community to preventhospital admission. You will assist the Independent Living Service Coordinator by undertaking duties required to meet the service users needs in order to re-enable service users to remain in their own home and prevent hospital re-admission. You will respondto referrals from Clinicians and Community Health Professionals and will be required to have the ability to assess service users needs, complete and follow a support plan for the individual. In addition, you will be providing practical and emotional supportin the service users own home and signposting to other services where appropriate to enable the individual to live independently in their own home. The Independent Living (IL) & Crisis Response (CR)service (IL/CR)supports people in the space between hospital and home and responds to the needs of people in crisis following an emergency; maintaining an extensive network of external relationships across health, social care, and emergency services. Working hours out with thenorm suits you.You live in greater Warmley and Bristol area and have a local knowledge of the geographical area.Also, you should be the following: Educated to GCSE level (or equivalent by experience) IT literate Full driving licence holder, with ready accessto your own vehicle (driver must be of legal age and have 2 years experience)Have a good knowledge of services provided by the NHS and Social CareThe closing date for application is 23.59 on Sunday the 5th of December 2021.
