Service Support Worker - Assisted Discharge Service Gloucestershire Royal Hospital and surrounding area Fixed Term Contract until 31st March 2022 24 Hours per week (2 day shifts, including one weekend out of 4) Salary: £16,217 per annum (pro rata) Wevegot an opportunity... As a Service Support Worker, you will provide the support and care which can make a vital difference to the quality of peoples lives, allowing them to continueliving at home in comfort. You will provide reactive, focused and flexible support to adults following a hospital stay to enable early discharge when clinical needs have been met. You will assist the Independent Living Service Coordinator by undertaking dutiesrequired to meet the service users needs in order to re-enable service users to remain in their own home and prevent hospital re-admission. You will respond to referrals from Clinicians and Health Professionals and will be required to have the ability toassess service users needs, complete and follow a support plan for the individual. In addition, you will be providing practical and emotional support in the service users own home and signposting to other services where appropriate to enable the individualto live independently in their own home. The Independent Living (IL) & Crisis Response (CR) service (IL/CR) supports people in the space between hospital and home and responds to the needs of peoplein crisis following an emergency; maintaining an extensive network of external relationships across health, social care and emergency services. You can deal with queries in a diplomatic and confidential manner.You love being flexible.
