Job Description

Local Area Coordinator (x 6 positions) Location: East Riding of Yorkshire covering each of the following areas: Bridlington, The Wolds, Beverley, Goole, Haltemprice and Holderness. Contract type: Fixed term (17 months - January 2022 to May 2023) Hoursper week: 35hrs Monday to Friday, 9am to 5pm each daySalary: £25,000 per annum (plus a £300 flexi home working allowance)Weve got an opportunity...An exciting opportunity has arisen for six outreach caseworkers to join the British Red Cross team in East Riding.You will be working on the innovative Local Area Coordination (LAC) programme, with individuals who currently, or may need to, access emergency or formal care. System pressures require a different way of working, with a focus on prevention and community development,so that sustainable outcomes can be achieved.Working across projects in six defined areas, you will deliver person centred, strength-based interventions that improve self-management, build independence and resilience, whilst preventing a need for formal care.To do this you will need to know some of these areas very well - Bridlington, The Wolds, Beverley, Goole, Haltemprice and Holderness (please specify areas in your supporting statement).You will be working intensively with a rolling caseload, helping peopleto identify and address unmet social needs that may be exacerbating physical and/or mental health. The people we support may have complex clinical presentations, (including mental health, substance misuse, multiple long-term conditions, trauma) and they maybe isolated and suspicious of formality and offers of support. So, we are looking for someone whose ability to connect is exceptionally strong. You must be authentic, relatable and compassionate, and able to think on your feet.You will also need to build strongrelationships with many partners, including adult social care teams, primary care, social prescribers, housing and the local voluntary sector. Together you will advocate for equity of access and improve integrated working.Lastly, as most of your time willbe spent out in the community; meeting people in their homes, in cafes or walking in the park, so you will also need to be confident of lone working procedures and able to keep meticulous support records whilst on the hop.
