The starting salary for this position is £21,943 per annum based on a 36 hour working week.Are you looking to develop a vibrant and diverse career in careAs an Integrated Reablement Worker for Surrey County Council you will find yourself on the frontierbetween health and social care, helping individuals focus on building their strengths and achieving their goals and independence.In your role for Mid Surrey Reablement, daily tasks will include supporting and assessing individuals within their own homes andstructuring personal support plans tailored to their lives. You will be working closely with families, health professionals and Occupational Therapists to ensure the people you help regain confidence. This role also provides an opportunity to develop leadershipskills by guiding more junior staff and carrying out observational supervisions.You will be reporting to Team Leaders regarding care plans created through assessments with the individuals in their homes and further ensure the plans are followed and completed.It will be your responsibility to oversee that the personal and health care needs of individuals are met with adherence to guidelines and procedures. Reablement works closely with the NHS and other health providers across all of Surrey providing hands-on carefor adults in their homes.
