Heres what you can expect to be doing You will work with a team focussed on the Intermediate Care Model, facilitating discharges from community hospitals or bedded care units and maximising independence so that individuals can remain in their own homes or communities.There may be a requirement to work a rota over 7 days per week with shifts covering the hours of 8am - 6pm.As Team Leader, you will work closely with your Locality Manager and your social work colleagues to lead a multi-disciplinary team, motivating and managing all members to ensure that their work is of a consistently high standard. Promoting independence should be the driving factor in everything your team does, and we’ll look to you to ensure that this is always the case.You will have the opportunity to shape how services in your area are delivered, designing the way that your team operates and bringing in creative ways of working. A few things about you You’ll be driven by your passion for making a genuinely positive difference to lives of service users. This will be backed up by your considerable knowledge and experience of Community Social Care practice - allowing you to provide clear direction and command the respect of your team.An integral part of this role is providing clear and inspirational leadership for your team - it is therefore essential that you have some experience of leadership or aspire to developing this. You should have a good understanding of data and be able to use this to evidence your practice.It’s also crucial that you have a real community focus and an eagerness to become embedded in the area that you’re serving. We’d expect you to be the expert when it comes to making community connections in your location with a comprehensive understanding of the local needs.You’ll be brave in your approach to work, not afraid to do things differently where it genuinely benefits the service user. You’ll be able to demonstrate a real creative spark when it comes to your work.
