Housing Support Worker£12phMonday-Sunday 8am-8pmDriving EsdsentialDBS on update service Build positive and effective working relationships and agreements with relevant external agencies- Progress and track referrals from the Local Authority into the service and outcomes for clients moving on from the service.- Delivery a high-quality off the street accommodation service for individuals rough sleeping within Mendip- Complete holistic assessments for clients leading to the development of support plans which meet individual client goals.- Implement person-centred packages of support with clients.- Give clients advice, information and provide an enabling service so that they can be signposted to and access appropriate services to develop independence- Provide an effective Housing Management service for clients and identified properties.If interested please call Josh or email
