We are looking for an Assistant Practitioner to join our pre-admissions team team. Practice Plus Group Hospital, Shepton Mallet has been rated Outstanding by the CQC*, putting it at the very top of the regulatorsquality ratings.You will be working as part of our expert and welcoming team, supported by our experienced Pre-Admission and Triage Lead Nurse. The key part of this role will be to ensure safe admission planning, ensuring that all admission requirements are in place priorto the patient arriving. As an Assistant Practitioner you will be responsible for the initial screening of all referrals received to ensure that they meet the admission criteria and you will assist the triage nurse with the ordering of x-rays and requestingof additional information from GPs and patients to support the process.About The CandidateWith an NVQ Level 4 in health and social care and previous experience gained in an acute healthcare environment, you will also have experience in patient case management.
