Job Description

Due to growth in our Continuing Healthcare department, we have an exciting opportunity for an enthusiastic experienced 2nd Care Coordinator to join our expanding South Yorkshire team. This team provides care and treatment for individuals, living within in theirown homes with complex care needs and in particular end of life care.Precedo Healthcare have been delivering Continuing Healthcare for over 10 years and hold contracts with local authorities as well as privately funded packages. We have an excellent reputation and pride ourselves in maintaining exceptional standards of care.Working within a small, multidisciplinary team and working closely with the Regional Manager the Care Coordinator will be required to:

  1. Prepare weekly staff rotas & update our internal computer rostering system
  2. Liaise with care workers on a daily basis to ensure smooth running of the rotas, and manage any potential issues
  3. Liaise with local authorities and private client families/representatives
  4. Attend local authority review meetings
  5. Carry out client risk assessments, reviews and spot checks
  6. Update client behavioural & care charts
  7. Assist in the induction of new care workers
  8. Process weekly staff payroll
  9. Complete local authority compliance paperwork
  10. General administrative duties
  11. On call as part of the Rota
  12. Cover care shifts if urgently required to
Our ideal candidate will have worked in Continuing Healthcare or a related care sector (e.g. domiciliary care) for at least 12 months. Ideally they will have previous experience of care co-coordination although this is not essential.You must also be able to demonstrate computer literacy, excellent written communication skills and have previous experience of administrative duties.Applicants must hold a full drivers licence, have access to a car and be able to travel throughout South Yorkshire.40 hours per week 5 out 7 - based at The Old PO Sorting Office, Dodd St, Sheffield S6 2NRPLEASE NOTE WE ARE NOT ABLE TO OFFER SPONSORSHIP
