We are looking for two enthusiastic, committed and motivated Social Prescribing Link Workers to join the Washwood Heath Primary Care Network (PCN) to support and empower individuals to take control of their health and wellbeing,Contract Term:One post is fixed-term for 1-year with the option to extend following review at the end of year-one.Second post will be for a fixed-term from date of appointment until 31 March 2021.The PCN comprises 9 General Practices working in conjunction with local health and social care organisations to improve the health and wellbeing of the local population. Washwood Heath is a Ward in Birmingham roughly two miles north-east of Birmingham city centre.Social Prescribing aims to address non-medical issues that may be causing or exacerbating long term health problems such as mental health, social isolation or have complex social needs which affect their wellbeing. The role is to adopt an holistic non-medical approach, co-designing a social prescription to improve health and wellbeing based on’ what matters’ to the individual and their individual strengths and needs and integrating services around the person.
