Job Description

Hours: 37.5 per weekThis an exciting opportunity to join our innovative, intergrated settings based in both the Community and Acute Trust.As a large, successful Foundation trust, we strive to continue to Vanguard a progressive way forward in Health and Social care and seek passionate, forward thinking, hard working individuals to join us in the challenge.The community environment is continually developing and due to recent major funding increases we are have the opportunity to offer the right candidates a position on our newly established rotation.You will gain opportunity and experience of working across a variety of services including Urgent Care/Rapid Response and Discharge to Assess.You will be expected to carry out excellent holistic assessments of patient needs and develop, implement and evaluate occupational therapy treatment programmes from a person centred approach. You will operate as an autonomous practitioner and key member of the multidisciplinary team and have an influential role in supporting and supervising junior staff and students. You will be responsible in supporting and coordinating discharges, leading complex case conferences and providing feedback to the team around patient pathways in aim to provide a seamless approach to patient care. You will have a degree or diploma in Occupational Therapy and be a member of the HCPC. You will be expected to have a broad range of experience managing medical, surgical, neurological, mental health and complex social care conditions and be able to address the impact these may have on an individuals ability to remain at home independently.
