Job Description

We are currently looking for a Band 7 Specialist Health Facilitator in the Sale area.Full time Monday-Friday. Temporary 7 month contract.You would be responsible for Management of clinical/professional input and agreed objectivesManagement of a clinical case load. Clinical Supervision of nursing and support staff (bands 6, 5, 4 & 3). Supervision and mentoring of student nurses. Take an active leadership role within nursing and wider clinical teamJob SummaryThe post holder will be expected to work as across verity settings and perform three main functions to meet the health need of Trafford residents such as (but not limited to):

  • Community Nursing
  • Health Facilitation
  • Hospital Liaison at Trafford General Hospital
The post holder will work in close collaboration with secondary and primary care providers; statutory organisations, private and voluntary sector agencies and commissioners to develop capacity and capability within health services to identify and effectivelyrespond to the health needs of people with learning disabilities in order to improve health outcomes and enhance the experience of healthcare services for service users and their families and carers.The post holder will raise awareness, develop skills and provide training to colleagues in primary, secondary and tertiary healthcare; statutory / voluntary sector services and education establishments to enable them to respond effectively to the healthneeds of people with learning disabilities.The post holder will work as a member of the specialist multi-disciplinary team, carrying out assessments and providing effective treatment, care and support to clients who may be experiencing health problems in addition to their learning disability, ensuringthose with additional complex needs are enabled to access healthcare to effectively and appropriately meet their needs via the provision of reasonable adjustments.The post holder will provide clinical leadership within the Trafford Community Learning Disability Team and will be expected to work autonomously, managing a defined case load of adult service users with Learning Disabilities and complex health needs, wherea high level of clinical expertise is required. The post holder will act as source of expertise and point of contact within the team and demonstrate highly specialised communication skills and problem solving abilities when prioritising risk.The post holder will hold a various caseload and develop an area of special interest in line with the 4 core specialist areas (challenging behaviour; physical health & PIMD; mental health: forensics).The post holder is expected to work in 3 core areas:
  • Direct clinical
  • Training and development of others
  • Project work to help develop services offered to people with learning disabilities
The post holder will provide clinical supervision to band 6 nurses, support workers and student nurses. The post holder will be expected to work flexibly in varying location and at varying times to meet the needs of the service.Qualifications- First level nurse (RGN or RNLD), Hold a relevant degree e.g. specialist practitioner or equivalent.
