Job Description

Job Purpose Responsible for managing the needs of clients and providing support to the wider team.The Role

  • Management of clients and responsibility for their ongoing care and support with a focus on person centred care.
  • Undertake consultation to new clients, complete assessments and complete all new clients paperwork as required.
  • Create and update client care plans and assist with digital care planning.
  • Audit care plans, paying particular attention to risk assessments, medication and moving & handling requirements.
  • Sign off logs and eMARs, identifying training needs and follow ups.
  • Maintain accurate client and CAREGiver records on software and People Planner.
  • Conduct client and CAREGiver introductions.
  • Build and maintain positive relationships with potential and existing clients and their families, resolving Client issues and queries, escalating to management when necessary.
  • Build relationships with other professionals involved in Clients care. This may include OTs, GPs, District Nurses, SALT teams etc.
  • Carry out regular client reviews and quality assurance visits.
  • Maintain regular contact with clients and CAREGivers.
  • Participate in on-call duties as required, typically one evening every other week and one day at a weekend approx once per month.
  • Ensure compliance with Home Insteads Equality, Diversity and Equal Opportunities Policy, in respect of employment and service delivery.
Carry out any other duties deemed necessary for thesuccessful operation of the business.
  • Be confident with safeguarding, complaints, accidents and compliment processes.
  • Work closely with CAREGiver team and Scheduling dept to ensure effective delivery of the service.
  • Work closely with Registered Manager regarding CQC regulations and National Office audits
  • Support business development function, identifying opportunities for marketing purposes.
Essential Criteria
  • Experience in the care sector delivering a wide range of personal care services.
  • Passion for delivering the highest quality care to our clients and helping them to live independently and happily in their own homes.
  • Identify everchanging client needs and increasing support packages as necessary, often at short notice.
  • Knowledge and understanding of legislation and regulations specific to Health and Social Care.
  • Good communication skills with the ability to build rapport quickly.
  • Must be confident to use care management technology including providing support and training to CAREGivers.
  • Must understand the importance of confidentiality working within current legislation.
  • Good working knowledge of IT systems with experience of Microsoft Office or Google Suite and virtual communication platforms with the aptitude to learn and adopt new technologies and software where appropriate.
  • Must have full driving license and means of transport if required within the territory to visit clients.
  • Be organised and flexible to meet the needs of the business with a focus on business growth.
Desirable Criteria
  • Level 3 NVQ in Health and Social Care or equivalent.
  • Experience of working with Electronic MAR sheets and medication in a supervisory role.
