Job Description

Care Group: LD, NDD and ABITeam: Trafford Community Learning Disability TeamRequired: Full time Mon-Fri - 08.30-16.30 ASAP - times are approximate and can be flexible - part time would also be consideredPlace of work: CLDT, 1st Floor, Sale Waterside, Sale, Manchester, M33 7ZF. (Having access to a car would support the worker. There is no parking directly outside Sale Waterside, parking is on local side streets or pay and display carparks (which would notbe refundable).Qualification: Registered as an Occupational Therapist with HCPC, would need a Degree or Masters in Occupational TherapyJob Role:

  • To have knowledge of working with people with a learning disability. Experience preferable but not essential.
  • To complete moving and handling risk assessments and advise on appropriate equipment needed and its use.
  • To liaise with hospital therapists and take the lead on the CLDT in supporting safe hospital discharge.
  • To complete mobility assessments and advise on appropriate equipment needed.
  • To carry out environmental assessments.
  • To carry out postural assessments and advise on appropriate seating and / or bathing equipment.
  • To work closely with CLDT Physiotherapist around 24 hour postural care.
  • To undertake occupational therapy assessments, addressing meaningful occupational performance, strengths and skills deficits, enabling the service user in areas of self-care, productivity and leisure.
  • To assess, monitor and manage risk in conjunction with the service user and those around them.
  • To collaboratively identify goals of the service user and provide occupational therapy intervention as part of the overall care plan.
  • To apply a high level of understanding of the effects of disability and occupational deprivation and provide training and advice on lifestyle changes as well as the service users physical and social environment.
  • To assess and actively manage individual, activity and environmental risk to ensure the safety of the service users, self, colleagues and wider public whilst recognising the right of the service users to take positive risks in promoting their recovery.
  • To plan and implement complex individual and / or group evidence-based interventions / programmes of care, in collaboration with the service user, using graded activity to achieve therapeutic goals. These interventions may take place in a variety of settingsincluding community and the service users home and / or day service.
