Job Description

Are you an enthusiastic pharmacist looking to develop your clinical knowledge and skills We are looking to recruit patient focused pharmacists with excellent communication and team working skills and self-motivation to contribute to the development and delivery of the pharmacy service.We provide patient facing pharmacy services with opportunities available in Sunderland. The pharmacy service operates Monday to Friday and provides a rotational emergency duty pharmacist service Trust wide.We welcome applications from pharmacist colleagues working in all sectors of the profession. Prior working experience in mental health is not essential, as a comprehensive programme of education, training and clinical supervision will be provided. We will support you to provide high quality services to patients and healthcare professionals.We provide nationally recognised pharmacy and medicines optimisation services to all our mental health, learning disability and neuro-rehabilitation inpatient areas, as well as to community mental health teams, crisis teams, psychiatric liaison and a variety of other specialist areas. Pharmacy staff are fully integrated into and regarded as valued members of the multi-disciplinary team. We have a number of pharmacist independent prescribers, who are active within a range of clinical services including dementia teams, STOMP clinics, Children and Young People services and community adult mental health services.Our clinical services include:

  • Medicines reconciliation and review on admission and discharge.
  • Supporting patients and carers with the management of their medicines
  • Optimising medicines, taking a holistic view of both mental and physical healthcare needs.
  • Physical health and wellbeing advice
  • Discharge planning and liaison with primary care
  • We take pride in developing our pharmacists through formal and informal study, clinical supervision and training. This includes offering opportunities to undertake post-graduate diplomas and the independent prescriber qualification.We are an award-winning team with a proven track record in delivering high quality, safe pharmaceutical services.
