Job Description

Social Prescribing Advisor - My Best Life BarnsleyTemporary, 18.5 hours, Fixed Term until March 2022An exciting opportunity has arisen for a part time advisor to join our team, fixed term until March 2022. Whilst we cannot guarantee extension, this is a possibility. Were looking for people who have a good understanding of health, social care and communityservices and how these services can help improve the health and wellbeing of customers.For our perfect candidate we are offering a flexible role, working in a caring and friendly team, with a great package of benefits.

  • £22,925 per annum, pro rata
  • 18.5 hours per week

More about the role:You will work closely with Health, Social Care and the Voluntary and Community Sector. Using your extensive knowledge in the community and voluntary sector, and your understanding of health and social care services, you will empower customers, give themtime, offer solutions, and ask what matters to you. Co-located across the borough (with a focus on the Dearne), you will work remotely using IT to keep in touch with your team and the community, keep accurate records and monitor service delivery and outcomes.Who you are:This role will involve gaining peoples trust and respect so you can work with them in their own homes and communities. You will be skilled at supporting people to identify and achieve their personal goals. You should have experience of working positivelywith people facing complex social and emotional challenges. You should have awareness of the wider determinants of health, such as debt, poor housing and physical inactivity, as well as other lifestyle issues and low-level mental health concerns by increasingpeoples active involvement with their local communities. You will have great interpersonal skills in supporting people, community groups and local organisations.Our ideal candidate will have:
  • Good understanding of Social Prescribing Services
  • Excellent communication skills and the ability to work closely with customers and professionals
  • A track record of working with statutory and community, voluntary sector organisations
  • Experience of working with vulnerable customer groups
  • Have a strong belief in strength-based support
  • Ability to work on your own initiative with self-motivation and drive
  • Good IT skills and ability to work remotely, keeping up to date and comprehensive records
  • The ability to work confidently and effectively in a diverse, and sometimes challenging environment
