Job Description

Social Prescriber - Rotherham Social Prescribing ServiceJob Type: 30 hours per weekSalary: £25,810 per annum (full-time equivalent)Location: RotherhamOur client is the lead body for supporting, developing and promoting the voluntary and community sector in the Rotherham borough. Its core activities and associated projects are a keystone for local community engagement and development. They are lookingto recruit another team member as part of an award-winning service.The RoleThe Social Prescribing Service works with GP Practices, Community Mental Health Teams and wider health services to support adults with long-term conditions and mental health issues to tackle barriers that prevent them from self-managing their health andwellbeing and having social and community connections. A key aspect of the role is Social Prescribing - to assessing the non-clinical needs of referred patients and, using a support planning approach, provide supported access to a range of support servicesand activities, primarily in the voluntary and community sector. This is not a Support Worker role. As such, the role does not involve providing ongoing or long-term face-to-face support for patients.The successful candidate will need to:

  • Be enthusiastic and able to motivate and influence others
  • Be able to build positive relationships and work in partnership with a range of organisations and individuals
  • Have an understanding of Social prescribing and the Rotherham voluntary and community sector
  • Have an understanding of issues affecting people with long-term health conditions and mental health issues and barriers to self-care and improved wellbeing
  • Have an understanding of the impact that social, economic and environmental factors have on health and wellbeing
  • Have excellent interpersonal skills
  • Work effectively as part of a team
  • Be able to write effective case notes and use data monitoring systems effectively
  • Understand the need for robust Information Governance including Data Protection and Confidentiality
  • Be resilient, creative and able to respond positively to change
An intrinsic part of the role is to travel throughout the Borough on a regular basis, therefore, it is essential that applicants have access to their own transport.
