The starting salary for this role is £29,291 per annum for working 36 hours per week. This position is offered on a 12-month secondment basis, please seek line manager approval before applying.Surrey County Council is determined to transform the way it works. Our Transition Team, based in Woking or Reigate, is a dynamic and vibrant countywide service for young people aged between 14 and 25. Although we sit in Adult Social Care, our journey with the people we support starts much earlier, at academicyear 9. Within Surrey County Council, the Transition Team sits in a unique and favourable position of being able to undertake frontline work across both Children and Adult Services.Are you passionate about ensuring adult carers and young carers get the support they need Are you looking for a new opportunity to make a difference for families that need social care supportOur Carer Practice Advisor position is an exciting and varied role that promotes good practice in whole family assessment and support planning within our Social Care Teams.A day in the life a Carer Practice Advisor may include:

• leading 1:1 and group good practice sessions with colleagues
• developing and delivering training to small groups within your team
• networking with education and local voluntary sector organisations
• presenting information sessions to public groups
• representing your team at local strategy groups
• supporting a colleague on a complex visit involving a parent carer
• delivering preventative, early identification workshops at carer support group sessions in our local communities and education settings
• reviewing your teams performance against defined carer and young carer performance measures
• developing an action plan to deliver improvements and best practice

You will work alongside the Transition Team as well as our area-based team of other Carer Practice Advisers over seen by our Senior Carers Practice and Development Lead to collaborate on new ways of working to deliver best practice and improve your teamsCarers performance. • Experience of working with carers and young carers in social care context
• Competence in leading and developing carers practice within the team
• Ability to support carers in complex situations through finding creative and dynamic solutions
• Good IT skills, and other MS Office applications with experience presenting to small groups

Working in this role, you will demonstrate sound knowledge of the Care Act (2014) and Mental Capacity Act (2005). You will work with internal and external partners to raise the profile of both adult and young carers.
