The starting salary for this role is£29,291 per annum for working 36hours per week.The Transition Team is a dynamic and vibrant countywide service for young people aged between 14 and 25. Although we sit in Adult Social Care, our journey with the people we support starts much earlier at Year 9. Within Surrey County Council, the TransitionTeam sits in a unique and favourable position of being able to undertake frontline work across both Childrens and Adults Services.Working in this role, you will be competent in leading and developing carers practice within the team. You will be a skilled individual who is able to support carers in complex situations through finding creative and dynamic solutions. Working with youngpeople with disabilities, you will demonstrate sound knowledge of the Care Act (2014) and Mental Capacity Act (2005). You will work with internal and external partners to raise the profile of both adult and young carers. As well as this, you will be skilledin strength-based approaches and ensure service delivery is outcome focused and evidences strong coproduction with carers.The Transition Team is a constantly evolving, fast paced service, where we like to keep challenging ourselves to achieve best practice and outcomes.
