Job Description

The starting salary for this role is £66,774 per annum, working 36 hours per week. About the roleAdult Social Care (ASC) is operating in a challenging environment and managing growing demand is key for creating a sustainable service for the future. This means we need to re-think our current model to ensure that we can manage demand and continue supportingour residents effectively in the long term.We are looking for an experienced social care practitioner to join us as a Senior Manager. Reporting to the ASC Transformation Programme Director and working closely with the Head of Integrated Reablement Services, ASC Operational Lead for Discharge, localitysenior managers and hospital social work teams, you will support the development of an out of hospital pathway model for the future. This model will have clear targets attached to it around managing demand. The role will include:
  • Moving to a preventative approach that supports home first on discharge and admission prevention, thus enabling people to remain in their own homes.
  • Contributing to existing work to shift the balance of our reablement offer to support people in the community, as well as hospital discharge.
  • Work with Surrey Heartlands ICB Place leads strengthen delirium and non-weight bearing pathways
  • Exploring innovate models to support hospital discharge, intermediate care and reablement that are financially sustainable and support the locality needs.
  • Defining and deploying a trusted assessor model for discharge.
  • Using health economics insight to inform future service demand and workforce capacity/skills mix.
  • To identify and implement key metrics that measure outcomes for people going through the pathways.
  • Working with Discharge To Assess integrated services, collaborating with Acute Hospitals and other partners, and supporting the ASC Operational Lead for Discharge
Whilst this position is countywide, we promote flexible and hybrid working with the expectation of up to two office days a week.About youWe are looking for someone with practice expertise and experience working in complex adult social care services. You will ideally have experience of reablement and hospital discharge in front line adult social care. You will need to evidence successful changemanagement, in a complex environment at scale - this role is all about driving transformation.You will be able to work with stakeholders at all levels to adopt creative new models rooted in a strong evidence-based approach. You will have knowledge of health economics to help model future service demands.
