Job overviewAn experienced nurse responsible for delivering nursing care to the practice population. Supported by senior nurses, they will deliver care within the boundaries of role and competences, focusing on supporting patients to be healthy, monitoring long term conditions, health prevention and screening activities. They will work within the nursing team, to meet the needs of the patients, supporting the delivery of policy and procedures and providing nurse leadership and teaching as required.To work within NMC Code of Conduct and to agreed practice standards and protocols.15 hours per week to be worked over 2 or 3 days.Main duties of the jobThe Practice Nurse works independently using own initiative and is accountable for their own professional actions. Provides support and leadership for other members of the Team.Administers adult and child immunisations and vaccinationsCompetent in anaphylaxis and resuscitation techniquesEnsures safe storage, rotation and disposal of vaccines and drugs and adequate stock levels are maintained.Undertakes procedures to include, venepuncture, glucose tolerance tests, BP management, urinalysis, peak flow, spirometry, stitch/clip removal, cytology, wound management, cryosurgery.Initiates the process of diagnosis with patients suspected to have a chronic disease eg Diabetes, COPD, Asthma, CHD referring to other clinicians as appropriate.Manages programmes of care for patients with chronic diseasesInitiates and carries out programmes of health screeningProvides healthy lifestyle and smoking cessation advice to individuals and groups.About usHolmhurst Medical Centre is an established General Practice serving over 10,000 patients in the Redhill area.
