Job Description

We are expanding our Community Intermediate Care Team and have a vacancy for a band 6 qualified nurse to join our dedicated team covering East Surrey.Working as part of a highly skilled multidisciplinary team you will need to possess excellent clinical and communication skills in order to provide comprehensive, timely and effective interventions, to assist in reducing unnecessary hospital admissions and support hospital discharge. You will assess plan and implement care whilst promoting independence to ensure patients can remain in their own home. As part of our role in facilitating early hospital discharges we provide intravenous antibiotic therapy to patients in their home.We are looking for an enthusiastic, flexible and dynamic individual who will be working with other qualified nurses and therapists in the team. ICT work closely with other teams within First Community and Social Care.You will also provide professional leadership for designated staff and will have a role in the leadership and line management of the wider multidisciplinary team.Experience in Community Nursing is desirable. The team operates seven days per week and the candidate will require a flexible approach to working including some weekends and bank holidays.Due to the nature of the role it is essential that applicants are able to drive, have a car suitable for business purposes and have a clean driving licence.First Community is committed to safeguarding and promoting the welfare of vulnerable adults and children and expect all staff to share this commitment. First Community will support personal development through the annual appraisal process and in house training.A 6 month induction course is mandatory
