Our client, a local authority; Bury is looking for a Social Worker for the Duty to Assess team.The post holder will be based at Heathlands Nursing Home, Prestwich, on the Discharge to Assess Team (D2A), our aim is to provide a social work service to the residents of Bury who have been discharged from an Acute Hospital setting. We are a multi-disciplinaryTeam consisting of, Occupational therapists, Physiotherapists, Technical Instructor, Social workers and a Social care officer. Although based at Heathlands and mainly assessing patients/ customers on the unit, the post holder will be required to assess patients/customers in other care home settings who have been placed in a D2A bed. Experience of Discharge to Assess social work or hospital discharge social work is essential, as well as the ability to manage your time and prioritise work loads.
