Greater Manchester Mental Health is one of the most active research trusts in the UK, providing one of the highest levels of clinical trials for mental health which puts the service users at the leading edge of new mental health treatments. The trust providesinpatient and community mental health services across Manchester with a wide range of specialised mental health services, having been branded as holding an "outstanding" substance misuse service.ABOUT THE SERVICEFor this role, the service is happy to offer remote working and potential for part-time candidates.Greater Manchester Mental Health NHS Foundation Trust (GMMH) offers a wide range of specialised mental health and substance misuse services across Greater Manchester, the northwest of England, and beyond, as well as inpatient and community-based mental healthcare for residents of Bolton, the city of Manchester, Salford, Trafford, and Wigan. GMMH also provide Health and Justice Services in a number of custodial settings.The secret to being able to deliver high quality patient care is having a skilled and adaptable workforce. To maintain the NHS standards, we therefore require more adaptable workers to or join our bank.To be successful, you will need to have:The post holder will work within an integrated, multi-disciplinary team within the Mental Health Acute Inpatient Wards in Park House and Laureate House, Greater Manchester Mental Health NHS Foundation Trust.The work is specifically support the MDT to plan discharge through the provision of assessment and planning of individual packages of care for service users and carers, in line with the Care Act (2014), Mental Health Act Legislation (1983) and Mental CapacityAct (2005). This will also require applications for funding, referrals to supported housing and support provision and overseeing this process ensuring that both service users, carers and MDTs are included and updated on these processes. There will be a particularemphasis on working through tasks associated with a service users discharge from hospital, and with avoiding readmission.The post holder will report to the Inpatient Spoke practitioner for day-to-day management. Professional social work supervision will be provided by a Senior Social Worker.The post holder will aim, at all times, to promote the independence and well-being of people; to assist social inclusion and valued community presence and to enable the widest possible expression of chosen lifestyle.Who are NHS ProfessionalsIf a hospital cannot supply staff on a ward or area, due to increased demand, sickness, or common staff shortages, they need to call upon Bank staff.
