Job Description

37.5 hours per week - permanent.Are you passionate about supporting people to live as independently as possibleAre you a fun loving, hard worker Where going the extra mile is the normAre you committed to and motivated about delivering therapy to help people achieve their goalsIf so, then we could be the team for you – a rare and exciting opportunity has arisen for an enthusiastic and forward looking therapy support worker to join our Multidisciplinary team.Tavistock Community Rehabilitation Team is based in the heart of a thriving community and in these challenging times, this forward thinking, award winning, Multi- Disciplinary Team has ensured that person centred care remains paramount.Tavistock Locality is proud of its’ integrated working practices between Health and Social Care, and the benefits it offers to our patients. Flexible working across all service areas will be an expectation of this post alongside opportunities to gain skills and experience within the wide spectrum of service provision – from community hospital to care homes to patient’s home.The work will include carrying out treatment programmes under the direction of the team Therapists, with patients on the ward at Tavistock Community Hospital, with patients in their own homes, in Residential or Nursing Homes, or when attending one of our Therapy groups. The post holder will be well supported by senior staff as well as the wider multi-disciplinary team, and will be actively encouraged to attend training and to engage in clinical supervision to enhance their continued professional development.We are extremely lucky to work in an area of outstanding natural beauty, based on the edge of the Dartmoor National Park. Many of our patients live in rural locations, so being able to drive and having access to a vehicle will be essential for this post.Livewell Southwest values its staff and makes investment in their development a priority.
