Up to 37.5 hours per week – permanent/secondment.We are looking to appoint an experienced, enthusiastic and motivated health care professional, with excellent assessment and communication skills, to join the NHS Continuing Healthcare and NHS-funded Nursing Care Team.Based at Windsor House, you will be responsible for assessment and review of individuals in their own homes and care homes, in order to identify appropriate NHS funding. You will be required to undertake some on-going care co-ordination of fully health funded clients with the support of a Band 5 Specialist Assessor. The role will involve assessment of people with complex health and social care needs and communication with clients and families regarding the application of the National Framework for Continuing Health Care. There will be an expectation that you will be able to work collaboratively across partner organisations and ensure that our clients are receiving appropriate care.You will have excellent verbal and written communication skills, have the ability to effectively organise, prioritise and plan your own workload recognising your limitations and seeking help when needed. You will need to be confident to work alone but also work well within a team and with other stakeholder agencies. A flexible approach to working in a pressured environment is essential.The ability to access all areas of Plymouth, Tavistock and the South Hams in a timely manner is essential.You will be well supported by the current team and receive a comprehensive induction to the post.
